Department/Unit - Sports Medicine
The purpose of the Initial Competency Evaluation is to confirm that
new employees and internal transfers have the minimum competencies
required to do the job and to verify compliance with behavioral
standards and expectations.
This form should be filled out by the leader and signed by both the
leader and employee within the employee's first 90 days in the new

Employee meets the minimum requirements of the following
Development and instruction of injury prevention programs in
collaboration with Athletic Directors, Sports Medicine Physicians,
Coaches and Therapists.
Communication with coaches and athletes regarding goals and
Recommend injury prevention procedures consisting of flexibility
and strength training.
Select and apply preventive taping, bracing and other equipment as
Recognition of injuries/conditions when they occur and refer the
injured athletic for medical care for further evaluation, diagnosis
and treatment, as appropriate.
Provide assessment of injuries as well as immediate care.
Appropriate first aid when required.
Advise coaching staff as to the nature and extent of injury.
Provide follow-up assessments to athletes.
Advise coaches on clearance for return to play based on physician
Assist in plans for rehabilitation and reconditioning of
Attend all games and practice sessions throughout the year
including summer months as requested by the athletic director.
Attend Sports Medicine interdisciplinary team meetings.
Actively participate in ongoing Sports Medicine strategic planning
including the development and implementation of an action plan to
meet strategic goals.
Cleaning of equipment used to render treatment to athletes on
school facilities will follow infection control policies and be
completed based on the cleaning policy.
Consult with coaches regarding appropriate topics, including the
selection and purchase of equipment, health maintenance, injury
Actively participates in establishing and maintaining positive
relationships with physicians, coaches, athletes, athlete parents,
rehabilitation therapists and other key customers.
Communicates with physicians, athletic directors, coaches,
rehabilitation therapists and others as appropriate about care of
athletes or problems that exists.
Family consultation in regard to athlete treatment and
Maintain athlete referral tracking logs.
Consistently strives to meet our customer satisfaction needs.
Responsible for ongoing evaluation of treatment for effectiveness
of treatment outcome.
Responsible for ordering, care and use of supplies and materials
including managing inventory.
Prepare a medical first aid kit for all coaches.
Consistently follows standards of service as defined by hospital
