Category Home Health Description Essential Job Functions:Staff/Team

  • Observe, coach, and monitor Program Coordinators and Management Team to assure consistency and compliance with ISP contract requirements, and agency policies not limited to the following areas:-Monitors and supports client documentation (ex. Review End of Month Documentation, ISP's, BSP's).-In collaboration with Quality Assurance Specialist/Health Service Coordinators ensures quality of all services and establishes systems and procedures to maintain quality and correct deficiencies.
    • Facilitate regular positive communication between all staff and team members.
    • Provide support to staff and team members with an open door policy, including the initiating of regular phone calls or meetings.
    • Complete performance appraisal documents, progress notes, and meetings for all direct reports as directed.
    • Provide support and training opportunities for direct reports.
    • As needed, initiate disciplinary action process in accordance to agency guidelines and in collaboration with Human Resources.
    • Attend and coordinate regular meetings with team.
    • Interview/Coordinate staffing for appropriate team members (Site Leads, Program Coordinators, other direct reports).
    • Create, track and maintain accurate and current caseloads.
    • Prepare for audits from the state/county.Client/Contract Review
      • Review client specific ISP, BSP (if applicable), and other relative services that a client is receiving or is eligible for to ensure compliance with contracts.
      • Ensures compliance with program contract requirements, agency policy and ensure completion of all reports as required (ex. ISP, BSP) and submit proper documentation such as signatures, dissenting opinions, changes, late memos to the file.
      • Ensure satisfaction and support given to clients, guardians, family, any member of support team in regard to complaints, grievances and appeals regarding client services, and escalates and effectively communicates as needed.
      • Attend all client related meetings as necessary (ex. provider interviews, ISP's, BSP's, and any other meeting).
      • Interact professionally and supportively with clients and peers within and outside of the agency.
      • Comply with and enforce all DSP job responsibilities as necessary.
      • Review and ensure End of the Month documentation is accurate and complete for each site/Program Coordinator.
      • Ensure LPS contract is followed, billable time is accurate.
      • Review and ensure health ad safety of each client by following up on each unusual incident/major unusual incident. Ensure/track that all follow up and review of reports has occurred. (ex, disciplinary action, medical follow up, team notifications, closures, etc.).
      • Ensure client transfers (incoming, outgoing) occur in a timely fashion between providers or within the agency by ensuring all aspects (move, start up, creation of client documentation, Advisor, ISP, HSC, etc.).
      • Assist with growth of Agency.
      • Advocate to protect and advance the rights of clients with disabilities so that they may freely exercise their own life choices, enforce their rights, and fully participate in their community life.
      • Work under the supervision of the CEO.
      • Embrace and embody the mission, guiding principles, and goals of Cap City.
      • Understand and remain current on all updates of duties and expectations as outlined in both the Ohio Revised and Administrative Codes under the following rules: (Full rule definitions can be found at http://codes.ohio.gov ).
        • 5123.62 Rights of Persons with a Developmental Disability (ORC).
        • 5123:2-13-04 Client Options Waiver/Homemaker Personal Care (OAC).
        • Other responsibilities as assigned.Essential Education, License, Certificates/Registration Qualifications:
          • Minimum of five years professional experience in the Developmental Disability industry or related field.
          • High School Diploma or equivalent.
          • Valid Drivers License (with no more than four points),
          • Current proof of state minimum auto insurance.
          • 21 years of age or older.
          • FBI and/or BCI Background (5123:2-1-05.1-ORC).
          • CPR and First Aid certified (5123:2-2-01-ORC).
          • Major Unusual Incident training (5123:2-17-ORC).
          • OSHA (Occupational Safety and Health Administration) training.
          • HIPAA (Health Information Protection and Accountability Act) training.
          • Certified in Medication Administration. Position Requirements Working Conditions:
            • Work is performed primarily in an office setting but traveling within Ohio is a frequent requirement.
            • On-Call availability (24/7) for Program Coordinator team and designated client sites. Must be able to respond to all inquires and emergencies via telephone in 15 minutes and in person within 30 minutes.
            • Must return all emails, phone calls and correspondence within 48 hours.Essential Physical and Mental Demands of the Job:
              • The employee must be able to perform the following essential duties and activities with or without accommodation.Physical Demands:
                • Requires mobility and physical activity: Having an adequate range of body motion and mobility to work in an office, residential, or outdoor environment including standing and walking (even and uneven surfaces), sitting for extended periods of time, bending, twisting, reaching, balancing, occasional lifting and carrying of up to 30 pounds. Use of computer and telephone systems is required which includes coordination of eye and hand, and fine manipulation by the hands (typing, writing, and working with files). Requires the ability to defend oneself and clients in physically abusive situations through the use of approved verbal de-escalation techniques only.
                • Requires talking: Expressing or exchanging ideas by means of the spoken word. Talking is required to impart oral information to employees, clients, patients, and the public, and in those activities in which the employee is required to convey detailed or important spoken instructions to others accurately, loudly, or quickly.
                • Requires hearing: Hearing is required to receive and communicate detailed information through oral communication.
                • Requires seeing: Clarity of vision at 20 inches or less and at distance. This factor is required to complete paperwork for many of the employee's essential job functions and to observe client behavior and activities in and out of the facility.
                • Appropriate personal protective equipment will be readily available at the site as needed.Mental Demands:
                  • Requires the ability to reason and be empathetic to client and direct report needs.
                  • Requires the ability to work and cooperate with clients, co-workers, managers, the public and employees at all levels in order to exchange ideas, information, instructions and opinions.
                  • Requires the ability to work under stress and in emotionally charged setting.
                  • The ability to defend oneself and clients in mentally/verbally abusive situations through the use of approve verbal de-escalation techniques.
