Job Description:
Our Hospice of South Central Indiana is looking to hire a new
Director of Clinical Services. We were founded in 1980 and now have
over 170 team members in 16 different counties in Indiana. If you

Expectations: 1. Serves as the Patient Family Care Coordinator for
the organization to ensure the overall coordination of care and
continuous assessment of each patient's and family's needs and the
implementation of the interdisciplinary plan of care. Serves as a
member of the Interdisciplinary Group.2. Oversees the day to day
operations of Home Services and Inpatient Services. Ensures that
hospice care is provided according to regulations and standards
established by Our Hospice.3. Accountable for planning, development
and management of Hospice Services to patients living in the home
setting (e.g. home, nursing facilities, assisted living
facilities).4. Accountable for planning, development and management
of Hospice Services to patients living in the Inpatient setting.5.
Accountable for the coordination of Inpatient Services to ensure a
high standard of quality care is provided to each patient,
consistent with the Registered Nurse and Home Health Aide /
Homemaker responsibilities on the interdisciplinary team.6.
Provides direction and oversight to the Patient Care Managers to
ensure that decision-making is in line with Hospice's goals and

for this position: 1. Education Bachelor's degree (BS); or
equivalent combination of education and experience.2. Experience
Five to seven years' experience in health care administration.
Previous experience in healthcare compliance program management
preferred.3. Licensure Licensed Registered Nurse in the State of
Indiana Valid Indiana Driver's License
