Job Description:
We are seeking a full-time Nurse Practitioner, including new
grads, for home risk assessments in Altoona, PA.
Conduct up to one-hour assessments of adult and geriatric patients
in various community settings, reviewing health, history,
medications, and social factors.
Collaborate with physicians, social workers, and caregivers to
improve patient outcomes and close care gaps.
Provide health education, diagnosis, treatment, referrals, and
follow-up, documenting according to protocols.
Work Monday to Friday, 8:00 am - 5:30 pm, with flexible hours
possible.
Salary ranges from $101,700 to $132,800 annually, plus incentives,
with benefits including medical, dental, vision, 401K, PTO,
holidays, and mileage reimbursement.
Requires Master--------s degree or equivalent, current RN and NP
licensure, certification, and at least 1 year NP experience.
Must be willing to travel up to 15%, including overnight stays,
with a valid driver--------s license.
Home risk assessments and community visits offer a meaningful way
to impact at-risk populations.
