The Hospice Nurse will work collaboratively with other members of the healthcare team to ensure that the patient's wishes and needs are respected.Responsibilities

• Assess and evaluate the patient's physical, psychological, and mental needs
• Educate the patient and their family on end-of-life care
• Develop and implement a plan of care that addresses the patient's needs
• Monitor the patient's progress and adjust the plan of care as needed

    • Assessments:Conduct comprehensive psychosocial assessments of patients and families to identify their emotional, social, and spiritual needs.
