invests in its employees!Hospice Nurse Job SummaryThe Hospice Nurse
will be responsible for assessing and evaluating the patient's
physical, psychological, and mental needs and providing nursing
services aimed at improving the quality of life for both the
patient and their family. The Hospice Nurse will work
collaboratively with other members of the healthcare team to ensure
that the patient's wishes and needs are
respected.Responsibilities
- Assess and evaluate the patient's physical, psychological, and
mental needs
- Educate the patient and their family on end-of-life care
- Develop and implement a plan of care that addresses the
patient's needs
- Monitor the patient's progress and adjust the plan of care as
needed
- Maintain patient records and provide written reports on the
patient's conditionRequired Skills and Qualifications

- Education: Graduate of an accredited school of nursing with an

- Certification: Certified Hospice and Palliative Nurse is a
plus

- Experience: 1+ years of nursing experience in a clinical care
setting (hospice or end-of-life experience preferred)Social Worker
(Hospice) Job Summary:The Hospice Social Worker provides
compassionate and holistic care to patients and families facing
end-of-life, offering emotional support, counseling, and resource
coordination to enhance quality of life during a challenging
time.Responsibilities:
- Assessments:Conduct comprehensive psychosocial assessments of
patients and families to identify their emotional, social, and
spiritual needs.
- Care Planning:Develop and implement individualized care plans
that address patient and family needs, collaborating with the
interdisciplinary team.
- Counseling & Support:Provide counseling and support services to
patients and families, including grief counseling and bereavement
support.Required Skills and Qualifications:
