Job Description: Provide health assessments, medical diagnoses of
acute chronic illnesses, write prescriptions and formulate
treatment plans. Requirements: Doctor of Medicine Degree or foreign
equivalent, Physician License from the State of Indiana, DEA
licensure and RX authority from the State of Indiana, Board
