Team Manager Division: Community Living Supervisor: QIDP, Service Coordinator Classification: Non-Exempt Job Type: Full-Time Pay: $13.50-$16.25 Our ideal Team Manager will be experienced in working with individuals with developmental disabilities, organized, self motivated, and have strong leadership skills. Team Manager's primarily work in the field with our clients, with some office time for administrative work. This is a great position for someone who likes to be on the go with each day being different! As a Team Manager, you will supervise our amazing Direct Support Professionals that work hands on with our clients. Essential Duties and Responsibilities: Monitor and direct progress on goals and provide monthly summaries to supervisor.
Review finances and budget monthly with supervisor.
Implement Individual Service Plans (ISP) as written; ensure communication to and appropriate implementation by all staff.
Ensure all requirements for certification and funding as appropriate are met: Medicaid, Social Security, and the Housing Authority.
Oversee medical issues for all customers, including medical appointments, pill counts, medication administration, ordering medications, pharmacy reviews, and all related documentation.
Documents (paper and electronically) and maintains individual client records (charts) related to health and safety.
Facilitate client to have community involvement through regular exploration and participation
Responsible for knowing and respecting each client's rights and choices.
Conduct and document required house drills (e.g. fire, tornado) and Health & Safety checklists
Conduct an on-site, face-to-face visit with each assigned individual at least once per month
Complete the Supported Living Monthly Checklist for each assigned individual and complete any follow-up indicated.
Ensure that home environment is healthy, safe, comfortable, and well maintained for customers and staff (e.g. contact landlord for repair issues, furnace filters) Ensure updated documentation in the setting including the resource books.
Ensures compliance for all state and federal regulations.
Provide regular reports to supervisor, including information on overall home management, unusual incidents, injury to staff or client(s).
Ensures staff provides timely documentation and reports as required.

Keeps current on all updates to policies and procedures of the position, attending staff meetings, and providing input to the Team Manager on issues as needed.
Lead and participates in staff meetings, as required.
Train and supervise DSP's.
Ensures adequate staffing maintaining service hours.
Ensures required training and professional development for all staff.
Reports to Service Coordinator staffing needs for assigned settings.
Attend all scheduled shifts.
Provides shift coverage, including emergencies, if no coverage is located.
Provide emergency pager coverage as needed.
Work a minimum of 15 hours of Direct Service per week.
Completes other duties as assigned. Qualifications:
High School diploma or GED preferred, Bachelor's degree in Psychology, Social Work, Special Education, Sociology, or related field a plus. Experience in providing services to individuals with intellectual or developmental disabilities preferred. A valid driver's license, proof of automobile insurance and acceptable motor vehicle report is required. Working conditions:
Hours of work are individualized to the client or setting supported and could include overnights, weekends, and Holidays. Team Managers will be required to travel based on the needs of the customer supported. Physical requirements:
Team Managers may be required to lift, stand, sit, squat, bend at the waist and knees, climb stairs, and participate in active treatment and programming activities. Standing may be required for a large portion of scheduled shifts and for long periods of time. When working with any individual, there is always the possibility of aggressive acts. Aggressive acts could consist of pulling, pushing, pinching, biting, hair pulling, kicking, slapping, punching, scratching, head butting, and throwing objects. Employees are responsible for wearing appropriate slip resistant footwear consistent with job requirements and using Agency-provided personal protective equipment as appropriate.
