Assignment Type: Long-Term (13 weeks) Overview: Ascension St.
Vincent is seeking an experienced Registered Nurse Case Manager to
coordinate interdisciplinary care plans for patients from admission
through discharge within an acute care hospital setting.
Responsibilities: Develop, coordinate, and evaluate comprehensive
care plans in collaboration with healthcare teams. Assess patient
needs, monitor medical necessity, and ensure appropriate level of
care. Facilitate care transitions and advocate for resources to
remove barriers to effective discharge planning. Collaborate with
physicians, social workers, and care transition teams to optimize
patient outcomes. Educate staff on documentation standards,
level-of-care criteria, and discharge resources. Requirements:
Licensure: Active RN license (IN or compact). Certifications: BLS
required. Experience: Minimum 3 years as an RN, with at least 2
years of case management experience in an acute care hospital .
EMR/Systems: Familiarity with Ascension systems and documentation
workflows preferred. Additional Details: On-call: No Start: ASAP
preferred Must complete Ascension onboarding, credentialing, and
health compliance requirements prior to start.
